Doctoral forms
This form should be included with the submission of any PhD that contains co-authored work.
A preliminary thesis proposal should be completed in consultation with your potential supervisor(s). This proposal should describe the research you are going to undertake.
This form is to be completed by the Head of Department (HOD), forwarded to the Faculty Postgraduate Dean and then to the Graduate Centre.
This form is for use when nominating the appointment of examiners for the Doctor of Medicine (MD) only. For all other doctoral degrees, please use the DOC3.
This form is for changes to doctoral registration conditions (suspension, extension, termination, permission to be absent, change in supervision).
At the time of submission doctoral students are required to bring this form to the Graduate Centre with the examination copies of their thesis. Note: This form must be signed by a Justice of the Peace (JP).

While researching their proposals or awaiting registration, intending doctoral candidates normally wish to access the University's Library. The completed form should be returned to the Graduate Centre and arrangements will be made for library access.
A candidate for the Doctor of Fine Arts Degree must apply in writing to be examined for the Degree within five years of initial enrolment.
At the time of submission Doctor of Fine Arts students are required to bring this form to the Graduate Centre with their written documentation for examination. Note: This form must be signed by a Justice of the Peace (JP).
