Doctoral forms
Important note
These forms are mostly for doctoral candidates who do not have access to Wahapū because they are enrolled in a named doctorate (DClinPsy, DMedSc, DHSc, DocFA, EdD) or are on the Old (2016) Statute.
If you can access Wahapū, you will not need to use the forms on this page, except for a few in the "Forms for all doctoral candidates" section.

These forms are for processes that are not embedded in Wahapū.

| Candidate Declaration on GenAI Use | NEW: From 1 September 2026 onwards, doctoral candidates must include this declaration with their full thesis proposal at confirmation, and with their thesis submitted for examination. Learn more. |
| Library Access Request Form | This form is for doctoral candidates who are no longer enrolled, but need library access to complete minor revisions (maximum 6 months). |
| Co-Authorship / Statement of Contribution Form | Updated in June 2026 |

| Doctoral Covid-19 Fee Waiver Application Form | This form is for doctoral candidates whose progress was affected by Covid-19 disruptions. You can only apply for this fee waiver of up to six months once you have passed 41 months of enrolment (or part-time equivalent). |

These forms are only for doctoral candidates who do not have access to Wahapū.

Forms for academic staff
These forms are for academic staff: mainly doctoral supervisors of doctoral candidates who are enrolled in a Named Doctorate. Completed candidature forms should be emailed to sgs-advisers@auckland.ac.nz, while completed examination forms should be emailed to sgs-exams@auckland.ac.nz.
