Doctoral Forms
Most of these forms are for Named Doctorates or candidates on the Old Statute who do not have access to Wahapū.
Important note
If you can access Wahapū, you will not need to use the forms on this page, except for the Co-Authorship / Statement of Contribution Form, the Co-Production Form, and the Library Access Request Form, which you may require when submitting your thesis or while doing revisions afterwards.
Forms for all doctoral candidates
These forms are for processes that are not embedded in Wahapū.

| Library Access Request Form | This form is for doctoral candidates who are no longer enrolled, but need library access to complete minor revisions (maximum 6 months). |
| Co-Authorship / Statement of Contribution Form | This form, which is included in the thesis, is used when a thesis contains co-authored work and/or contributions from others (whether the work is published or unpublished). |
| Co-Production Form | This form accompanies the submission of any doctoral thesis that contains co-produced creative practice components. It is available on request from sgs-exams@auckland.ac.nz. |

These forms are only for doctoral candidates who do not have access to Wahapū.

| Doc 9 | Intention to Submit the Written Component for a Doctor of Fine Arts Degree (DocFA). |
| Doc 9a | Declaration to Accompany the Written Component for a Doctor of Fine Arts Degree (DocFA) on Submission. |
| Thesis Embargo Form | Named Doctorates and those on the Old Statute can apply to embargo their thesis by completing this form. |

These forms are only for academic staff, mainly doctoral supervisors.

| Doc 3 | Nomination for the Appointment of Examiners for a Doctoral Degree (Old Statute and Named Doctorates except DMedSc/MD). It is completed by the Main Supervisor. |
| Doc 3a | Nomination for the Appointment of Examiners for the DMedSc/MD Degree. It is completed by the Main Supervisor. |
| Doctoral Oral Examination Report | A summary of the candidate’s oral examination performance, and the examiners’ recommendation. It is completed by the Independent Chair. |
| New Supervisor Accreditation Form | Academic Head Confirmation of Conditions of New Supervisor Accreditation Form. It is available on request from sgs-advisers@auckland.ac.nz. |
